Wow. That fortune was meant for me. I was totally writing a post about all I have done recently in my career and how it's not fair that I was rejected for the second year in a row for something which, in my opinion, I am TOTALLY qualified to receive.

Oh no. I submit my paperwork for that just after we get back from Arizona. I won't know about the results of that until June.
If I don't get it, which is unlikely, I will have 6 months to find a new job. The policy here is up or out. If I don't get promoted, I don't have a job.

Doesn't this system lead to a tremendously top-heavy organization, or does the "promote or die" theme change at a certain level?

It's an up or out procedure for the first two levels.
After you are an L3, you can't be fired - kind of like a tenure system.
There are additional library levels you can go for after that point, but rejection won't have an effect on your ability to stay at your current position nor is it mandatory.
